Alternative Test Request Form (ATRF) Available Online
The ODA Test Center Alternative Test Request Form (ATRF) is now available online. Paper forms will no longer be supplied by the office. Students can now download the form from the ODA website (see below) or go to https://disability.unt.edu/testing-center. Beginning Fall 2018, the Alternative Test Request Form (ATRF) can also be submitted via the ODA testing email testing.oda@unt.edu . When submitting the Alternative Test Request Form (ATRF) please include student's name and the acronym ATRF in the subject line, or there may be a delay in scheduling. Please refer to our website https://disability.unt.edu/testing-center for more detailed instructions. If you have any questions please feel free to contact our office at (940) 565-4323.
* Students MUST request letters of accommodations (LOA) each semester AND give them to each instructor BEFORE this form can be completed and submitted to ODA.
