Filing a Claim

The injured employee must immediately notify their supervisor of any incident or injury and submit the appropriate paperwork to Risk Management Services (RMS) within 48 hours even if medical treatment is not needed.  Failure to report immediately may jeopardize a claim for workers’ compensation.

Once RMS is aware of the injury, the injured employee and supervisor will be emailed the Workers’ Comp Supervisor Packet and the Workers’ Comp Employee Packet to complete. For incidents and injuries that require no medical treatment, the supervisor must complete the Workers’ Comp Employee Injury Report Form.
