This form must be submitted to the TAMS Academic Office any time you plan to miss class or if you have missed class for any reason. For unplanned absences, the form must be submitted within 36 hours of the absence. If you are ill and unable to attend class, you should contact your parent/guardian immediately and also seek an appointment at the UNT Health Center. In order for the absence to be excused for illness, your parent/guardian must contact your academic counselor by 5pm on the date that the absence occurs. Be aware that faculty will also be reporting absences. If you have failed to report an absence that is reported by your instructor you will receive additional consequences for your failure to attend class.
