Catastrophic Care Benefits

OptionalLife Insurance

 

Optional life coverage can be purchased up to four times the annual salary with a maximum of $400,000. Coverage premiums are based on your age and level of coverage.  You can purchase 1 or 2 times your salary during your first 30 days of employment with no questions asked.  If you purchase 3 or 4 times Evidence of Insurability is required.

Voluntary Accidental Death and Dismemberment Insurance

 

This benefits offers protection to you or your family in the event of an accident based on the level of coverage you choose between $10,000 and $200,000.

Disability Insurance

Short-term Disability Overview

Once a participant's claim is approved, short-term disability provides 66% of your monthly salary with benefits payable for a period of up to five months (a maximum of 150 days) after completion of your 30-day waiting period or use all available sick leave (including extended sick leave, donated sick leave and sick leave pool), whichever is longest. To be eligible for benefits, you must provide a physician certification that you are totally disabled and unable to work.

Long-term Disability Overview

Once a participant's claim is approved, long-term disability provides 60% of your monthly salary. Benefits are payable until you are able to return to work or until you reach your Maximum Benefit Period (based on the age you become disabled) or based on the condition causing your disability. You must complete the 180-day waiting period or use all available sick leave (including extended sick leave, donated sick leave and sick leave pool), whichever is longest before you can be approved for a claim. To be eligible for benefits, you must provide a physician certification that you are totally disabled and unable to work.

Dependent Term Life

This coverage offers $5,000 of term life insurance for all your eligible dependents for $1.38 per month.
