Faculty & Staff Forms

The following forms are available to CVAD Faculty and Staff. Submitted forms will be sent to CVAD IT Support. Please do not submit multiple forms for the same issue. If you have questions regarding a submitted form or about any of the following forms, please email us at artsupport@unt.edu.

Submit a Helpdesk Ticket (Faculty, Staff, Students, and Guests)
If you are a current CVAD Faculty or Staff member or current CVAD student, we are happy to provide you with IT support. Using this ticket, you may report an issue, report a new hire, schedule a consultation, submit content for web and/or social media posts, or request hardware/software, etc.

CVAD Faculty Enhanced Privileges Agreement (Faculty)
The College of Visual Arts and Design recognizes that the research responsibilities of its faculty require that elevated privileges be granted to them on their personally assigned computer(s) and\or grant-funded equipment. After submitting this form, your information will be reviewed and consideration will be given.

Equipment Checkout Terms of Agreement (Current Faculty, Staff, and Students)
The CVAD IT Services Equipment Checkout service current faculty, staff, and students to check out state-owned equipment for university use (academic or professional) for a set amount of time, free of charge. Review the following terms carefully - by using this service, you signify your agreement to these Terms of Agreement.
