Guidelines for Completion:
Please log in with your EUID to use the webform.
Forms are due by Tuesday evening of the week of your requested performance date.
Faculty approval is required on all forms. The webform will automatically email to your private lesson instructor and Mr. Ford.
Forms that do not have all the information completed and accurate will not be accepted. This includes the year the composition was written as well as the birth and/or death year(s) of the composer. If your composer is living and you cannot find their birth date you can email them and request it. Also make sure you have the correct title of the piece listed.
Mr. Ford will determine performance order.
