Request to Receive a Certificate of Completion

Please review the requirements for the Graduate Academic Certificate you are requesting a certificate of completion for:

If you have completed all of the courses required, you may submit the form below to request your Graduate Academic Certificate. Once submitted, your transcript will be checked for confirmation and then the proper administrative paperwork will be sent to the UNT Graduate School. The Graduate School is responsible for sending your formal Certificate via U.S. Mail in approximately twelve weeks. The Academic Certificate credential will also be posted on your UNT transcript.

Please do not complete the form until the end of the semester in which you take your last course required for the certificate.
