Summary

To be eligible to enroll in the employee graduate student insurance program you must be employed in a salaried position.  That position must be at least half-time (20 hours per week). To confirm eligibility, contact the person responsible for employee timekeeping in your department.

If you are a Benefits Eligible TA/TF please review the PowerPoint Presentation below.  Once you have viewed the presentation, review all other information linked below.  Once you have completed the necessary documents, come by our office in Marquis Hall, Room 150.  We will review your documents for completion and enroll you in your benefit selections. All eligible TA/TF's are automatically enrolled in a waived status until completion of the necessary documents.  Failure to complete and return the packet can prevent an employee from obtaining benefits.

Available Options

If you are eligible to enroll, health insurance is free for you!  There is no charge for member-only coverage. Coverage begins the first day of the month following your 90th day of eligibility. You MUST enroll in health insurance to participate.  We cannot automatically enroll you.  Either complete and hand in the insurance information form in your packet or enroll online. If you do not enroll in health insurance during your first 90 days of eligibility, you may be required to prove good health if you want to join later.

Other available options include flexible spending accounts as well as the following insurances: dental, optional term life, dependent term life, voluntary accidental death & dismemberment (AD&D), short-term disability, long-term disability, and long term care.
