You must submit this form each semester you are planning to take courses. By submitting this form, your request will be sent to your advisor for review. Submitting this form early in the semester before you plan on taking courses is strongly encouraged.

Technology requirement

All students must select one of the options below.

By selecting one of the options below, I acknowledge that I have met the technology requirements for my program of study.

Requesting advising clearance on the following courses:

What does this mean?
  • Status – defines the action you want to do. For example, if you are requesting to take the course, you would select Add as the status. If you have previously been advised on a course, but are not planning to take that course, you would select Drop.
  • Subject – determines what college/department that you want to take the course in. For example, if you are taking a course in Library, or Information, Science, the subject may be either SLIS, or INFO.
  • Number – determines the actual course that you want to be advised on. For example, you may want to take SLIS 5000, Information Professions. See the Schedule of Classes for more information.
  • Section – determines which section of the course you selected to be advised on and that you want to be in. In the course example for SLIS 5000, Information Professions, you may want to be in the face–to–face version of the course which in the Spring has been section 002. See the Schedule of Classes for more information.
  • Instructor – indicates the name of the instructor for the course and section. See the Schedule of Classes for more information.

Important Notes:

Students are responsible for knowing catalog requirements and preparing a degree plan with their advisor prior to enrollment beyond 12 semester hours credit.

All advising requires signed approval of the Advisor and Associate Dean (Master’s Students) or Associate Director (Ph.D. Students). Upon review, changes and conditions may be specified by the Associate Dean, or Associate Director whenever needed according to the student’s status, class enrollment limits, program conflicts, etc.
