"IMPORTANT
NOTICE: Only those
persons registered by DEA to manufacture Schedule I or II
controlled substances may utilize/submit applications for
manufacturing/procurement quota for those controlled substances.
Only those persons registered by DEA to import the List I
chemicals ephedrine, pseudoephedrine, or phenylpropanolamine,
including drug products containing those List I chemicals, or
persons whose requirement of registration to import those
chemicals is waived by virtue of their registration to import
controlled substances, may utilize/submit applications for import
quota for those List I chemicals.
Only those persons registered by DEA to manufacture the
List I chemicals ephedrine, pseudoephedrine, or
phenylpropanolamine, including drug products containing those List
I chemicals, or persons whose requirement of registration to
manufacture those chemicals is waived by virtue of their
registration to manufacture controlled substances, may
utilize/submit applications for manufacturing/procurement quota
for those List I chemicals."
The forms maybe
completed :
1. As an interactive form which can be
completed on-line and printed on your printer, or
2. As a blank form to be printed on your printer and completed by
hand. This
second version is not recommended; fewer errors occur if the form is
entered on-line and then printed.
