- AZ, CT, FL, HI, MN, PA, PR, RI, SD, TX, and UT did not respond, or provided only a limited response, to the reporting population questions in 1991 through 1994
- NJ and NM did not respond, or provided only a limited response, to the reporting population questions in 1991 through 1993
