My colleagues and I developed a readability toolkit that includes an "alternative wording list" of ~ 625 words. It is freely available to download from this web site:

We refresh this list periodically, and always welcome suggestions for word to add (either complex terms that need easier synonyms, or additional plain language synonyms for the words already on the "difficult" list. Within the toolkit there are other resources, including links to other sites that decode/define medical jargon.
