If you wish to have a retirement plaque manufactured, please fill out the plaque request form. There are several message formats from which you can choose, or you may customize it. Keep in mind the limited space you have to work with and font size. It is recommended you use size 12 font or larger. Font size smaller than 12 takes away from the appearance of the plaque, it also causes poor definition in print quality. If possible, please submit multiple plaque requests at one time so that the PhotoFabrication Shop can more efficiently fulfill your order.

The retirement plaque is a benefit paid for by the Lab. The value of these handsome retirement plaques is about $150. Work will begin on the plaque only when your request form is received.
