Great News! The new Family Planning Benefit Program offers:
- annual gynecological exams
- birth control
- pregnancy, STD/HIV testing for family planning patients
Male and Female services are provided.
Do you qualify? Chances are that you do.
- Make an appointment with our friendly staff: (716) 858-8422.
- Bring the following information when you come to clinic:
- proof of who you are
- proof of home address
- social security #
- insurance card (if you pay)
- proof of income (wages, child support, military)
- proof of citizenship or alien status
- proof of payment if you pay for child support
*Teenagers ages 13-20 only need photo ID & Social Security #.
