APPLICATION FOR PRIVATE PESTICIDE APPLICATOR LICENSE


Instructions



If you would like to complete, submit, or pay for a Private Pesticide Applicator License, you will need a Private Applicator ID before you begin.
  1. This is obtained from the Colorado Department of Agriculture once you have successfully completed your Private Pesticide Applicator Exam.
Fields with a red asterisk '*' must be filled out before you are able to continue.

To get started, locate the *Applicator ID: text box located on the lower left hand side of the browser.
  1. Enter your five digit applicator ID.
  2. Click 'Continue'.

You will be directed to the Applicant Basic Information page. This page collects all personal contact information relevant to your license.
  1. Please fill out the Contact Information section
    1. Date of Birth will require the mm/dd/yyyy format (ex. 01/01/1970).
    2. Phone Number will require the single dash format (ex. 303-555-5555).
  2. You will need to fill out the Mailing Address section if your mailing address is different from the Records address. If the address is the same, select the checkbox and the fields will automatically populate.
  3. Select the Agricultural Commodities that apply using the check boxes at the bottom of the screen. More than one box can be selected. If you select 'Other', you must specify the commodities in the text field next to the 'Other' check box.
  4. Once you are finished, click the 'Continue' button.

You will be directed to the Licensure Questions pages. There are two parts to this section.
  1. You are required to honestly answer all questions.
  2. If you answer 'Yes' to questions 2-8, you are asked to provide additional detail in the text boxes below.
  3. Once you are finished, please click the 'Continue' button.

Please verify that all information on the Confirmation Page is correct.
  1. You may click 'edit' to correct any information you have provided.
  2. If all information is correct, please click 'Confirm & Continue'.

On the Attestation Statement page, you are required to electronically 'sign' the agreement, which confirms that the information you have provided is correct and accurate to the best of your knowledge.
  1. Please enter your full name in to the 'Name' field.
  2. Click 'Continue'.

Select the Credit Card or eCheck option.
  1. Click 'Continue'.

Enter the payment information:
  1. If using an eCheck, select an account type and enter the routing number and account number for the account. Enter the account and routing numbers a second time at the bottom of the page to ensure accurate entry.
  2. If using a credit card, select the card type (American Express, Visa or MasterCard only); enter the card number and the expiration date in MM/YYYY format.
  3. Click 'Continue'.

On the Payment Information Confirmation page, review all data for accuracy. If there are any changes to either the Payer Information or the Account Information sections, click the appropriate 'Edit' button.
  1. To complete the transaction click 'Agree/Continue'.

Once the transaction has gone through successfully, the Receipt page will appear. To print a copy of the receipt click 'Printer Friendly Version (PDF)' at the bottom of the page. A receipt will also be emailed to the email address you provided.

If you would like to pay for another license, click 'Back to Start Menu' to return to the beginning of the Private Pesticide Applicator License application.
