Community Action Grant Program

Guidance for Applicants (GFA) No. PA 00-003

The Community Action Grant Program supports the adoption and implementation of exemplary practices related to the delivery and organization of services for children with serious emotional disturbances or adults with serious mental illness. The target population may also have co-occurring disorders, such as substance abuse or other mental, emotional, or behavioral disorders. This program is made up of two types of grants: Phase I grants focusing on consensus building and decision support, and Phase II grants focusing on implementation support.

In 2001, the standing announcement has been revised to correct typographical errors.

SAMHSA applications consist of three parts:
1. "Part I—Programmatic Guidance" of the Guidance for Applicants (GFA) provides details about the specific program.

2. "GFA Part II" gives standard provisions for SAMHSA applications.  However, not all GFAs require Part II.  The latest edition of Part II is always available by mail or for electronic downloading from this website. 

GFA Part II is required for this GFA; click here for more information and to download a copy of the file.

3. SAMHSA applications require forms PHS 5161 and SF-424.  These forms are available online from the Program Support Center of the U.S. Department of Health & Human Services.
