Slide 1

  • Patient and Family Centered Care (PFCC) is associated with high patient satisfaction and positive outcomes for Total Joint Replacement Patients.

Slide 2

The Innovation

  • The PFCC model at Magee-Womens Hospital of University of Pittsburgh Medical Center (UPMC) is a comprehensive, systems based approach that focuses on meeting the multiple, on-going needs of patients and families through the full cycle of care. The initiative, which was piloted within the Total Joint Replacement program, is organized around the patient's experience, including pre-surgical care, the actual surgery and post-surgical intervention and rehabilitation.

Slide 3

What is PFCC?

  • Definition:

PFCC, Patient and Family Centered Care, is a redesign of patient care so that the hospital resources and personnel are organized around the needs of the patients and families rather than around various specialized hospital departments. PFCC is a methodology to achieve the goal.

Slide 4

PFCC Methodology

  • A new approach to the care of our Patients.
    • It is a process and methodology.... It is a mind set and the culture!

Slide 5

The PFCC Journey will result in...

  • Providing unique Patient and Family Experiences—not just services.
  • Adaptive Learning Environment.
  • Branding and Differentiation derived by the "experience."
  • Dream Health Care Team.
  • A Focus...a Passion...a Team and Health Care Champions!

Slide 6

The slide shows a cartoon of three children ready to board a school (Omni) bus. Each child is shown asking the bus driver either "Quality?", "Meds?", or "Safety?" The bus driver responds with, "Hop on! It's all on my route apparently..."

Slide 7

  • Changes made possible by the PFCC Methodology within the Total Joint Replacement Program at Magee-Womens Hospital of UPMC.

Slide 8

"Wellness" Rooms

The slide shows a photograph of a nurse standing next to an empty hospital bed in a typical "wellness" room.

Slide 9

The Family Room: A Sense of Community

The slide shows three photographs: one of two types of chairs to relax in; one of a computer station area; and one of the entire sitting room.

Slide 10

Gym with the Health Club Feel to support Group Therapy

The photograph shows patients and staff performing therapy in a large, open room.

Slide 11

Safe

The bar graph measures the "Mortality Rates" at 0.1% and "Infection Rates" at 0.3%.
  • Zero dislocations.
  • Surgical Care Improvement Project (SCIP) compliance: 98% for antibiotics within one hour of surgery.

Slide 12

Effective

The bar graph measures "Patients discharged without handheld assistance directly to home" at 91.0% and "Patients reported that pain was not an impediment to PT [physical therapy] (including same day of surgery PT)" at 99.0%.

Slide 13

Patient Centered

The bar graph measures "Press Ganey Mean Satisfaction Score" at 91.4% and "Positive Responses to "Would refer family and/or friend'" at 99.7%.

  • 99th National Percentile Ranking.

Slide 14

Efficient

The bar graph measures "Total Knee Replacement" for total joint replacement (TJR) patients average length of stay (ALOS) at 2.8 days and for National ALOS at 3.9 days. "Total Hip Replacement" for TJR Patients ALOS is 2.7 days and for National ALOS, 5.0 days.

Slide 15

Exporting the PFCC Methodology

The PFCC methodology can be exported to any Care Experience, not just TJR. The following is a list of active working groups and areas in which PFCC has been successfully exported:

  • Orthopaedics.
  • Trauma.
  • Day of Surgery.
  • Bariatrics.
  • Rheumatology.
  • Human Resources.

Current as of January 2009
