Forms can be filled in and printed directly from Acrobat Reader 3.0 or later. For assistance, see the Fill-in Forms Instructions. Practitioners who register for Practitioner Access and consent to receive electronic service are eligible to electronically file (eFile) the following fillable forms: "Entry of Appearance", "Notice of Change of Address", and "Substitution of Counsel". The "Notice of Change of Address" is the only fillable form that petitioners who register for Petitioner Access and consent to receive electronic service are eligible to eFile. Before electronically submitting a fillable form, the eFiler should fill in the form using Adobe Reader or equivalent viewer and then print (rather than save) it to PDF or export it to TIFF so that the data is embedded in the document. If an eFiler submits a PDF form before the data is embedded, the Court will receive a blank document and reject it. |
