KYPHOPLASTY FOR TREATING VERTEBRAL COMPRESSION FRACTURES WITH KYPHOSIS.

Objective: A full systematic review was performed on behalf of the Medical Services Advisory Committee to ascertain whether there was sufficient evidence on the safety and effectiveness of kyphoplasty compared to conventional surgery to recommend public funding of the procedure. Methods: All available literature on kyphoplasty was searched by means of nine bibliographic databases, ten other electronic databases, websites of osteopathy/neurology associations and hand-searching of relevant journals. Potentially relevant literature was retrieved, and studies were included if they addressed the population, therapy, comparator, outcomes, study design, search period and language delineated in the protocol. Included studies were pearled for further references. The best available evidence was used, and studies were rated on their level and quality of evidence, as well as clinical relevance. Results: No comparative studies were found that compared kyphoplasty against conventional surgery. A total of 26 original studies were included. Most of the studies were of low methodological quality with no treatment comparator. Therefore conclusions on the safety and effectiveness of kyphoplasty were tentative and the Medical Services Advisory Committee recommended there was insufficient evidence to fund the procedure. Discussion: Systematic reviews can be a valuable tool in assisting decision makers with funding decision concerning new health technologies, but higher quality primary research is required before kyphoplasty may receive funding in Australia.
