DESCRIPTION

This measure is used to assess the percentage of hospitalized adult patients who require hospital readmission within 30 days of discharge for conditions related to venous thromboembolism.

RATIONALE

The priority aim addressed by this measure is to reduce the risk of complications from pharmacologic prophylaxis.
