Frequently Asked Questions (FAQ)
- Question: Why can't I submit the application?
Answer: Review the HRSA 99-5 and verify that all pages of the application are
marked "Yes" for completion. Verify that the answers to the bottom 5 questions
are answered correctly. If any of these are answered incorrectly, go back through application
and review/correct/update and be sure to select "save and calculate" for any
page in question.
- Question: Why are the invoices and letters going to a new person?
Answer: The invoices and letters are mailed to the person you have entered on
line #2 of the HRSA 99.
- Question: Where can I find a copy of the Rules of Behavior to give to my replacement?
Answer: In the footer section click on the link for Security and Privacy Policies.
Scroll down to the bottom of the page and click on the ROB link.
- Question: The check mark, on the HRSA 99 for using the same address from above, is not
showing on my printable copy, is this a mistake?
Answer: The check mark is only to help with data entry, it is not part of the form and
therefore will not print.
- Question: Why, after you save, are the input values aligned differently than the other
line items?
Answer: This was done so that the user could easily see what cells needed to be
filled in by them and what cells would calculate.
- Question: Who do I contact if I have a question, an error on the system or a problem
I cannot solve?
Answer: Please call your regional manager first. If there is a computer glitch or
error on the system, please save the information or do a print screen that can be sent to
HRSA. This will help to fix any issues much faster.
- Question: Do I have to fill out the survey?
Answer: Yes, this is one of the ways we can improve the system. There is a general comments form
that can be used at any time.
