New TIP: Incorporating Alcohol Pharmacotherapies Into Medical Practice

Treatment Improvement Protocol (TIP) 49: Incorporating Alcohol Pharmacotherapies Into Medical Practice provides guidelines for the use of medications in the treatment of alcohol use disorders. Written for physicians and other health care providers, the TIP presents information on the four medications approved by the Food and Drug Administration to treat this disease: acamprosate, disulfiram, oral naltrexone, and extended-release injectable naltrexone. The TIP describes each medication’s mechanisms of action, proper dosing, side effects and their management, indications and contraindications, and patient management.

Report Shows Substantial Disparities in the Levels of Substance Abuse and Mental Illness Problems

A new report providing state-by-state analyses of substance abuse and mental illness patterns reveals that there are wide variations in the levels of problems like illicit drug use found among the states, but that every state suffers from these problems. For example, among those aged 12 and older, Iowa had less than half the current illicit drug use rate of Rhode Island (5.2 percent vs. 12.5 percent) – yet Iowa’s population aged 12 and older also had one of the nation’s highest levels of people experiencing alcohol dependence or abuse in the past year (9.2 percent).

A new report which coincides with Children’s Mental Health Awareness Day reveals that 8.2 percent (2 million) youths aged 12 to 17 experienced at least one major depressive episode (MDE) in the past year. Only about two-fifths (38.9 percent) of these adolescents received treatment during this period according to the report by the Substance Abuse and mental Health Services Administration (SAMHSA).

A study on Housing First found a $4 million decrease in the use of crisis intervention services over the course of 12 months. The study, Health Care and Public Service Use and Costs Before and After Provision of Housing for Chronically Homeless Persons With Severe Alcohol Problems, was published in the Journal of the American Medical Association earlier this month (JAMA, 301(13):1349 – 1357). The research took place at 1811 Eastlake, a Housing First program within Seattle’s Downtown Emergency Services Center (DESC).

Following major disasters, the existing public health infrastructure is often fragmented or completely destroyed, as was evident following the Indian Ocean tsunami of 2004 and Hurricane Katrina in 2005. In an effort to address this fragmentation, and based on previous experience working with the United Nations Children’s Fund, the U.S. Agency for International Development, Louisiana Department of Education, and Texas A&M Health Science Center developed and implemented what is known as the Mercy Model. The Mercy Model is a method or set of precepts describing the facilitation, organization, and leadership of systems following a disaster. At its most basic level, the Mercy Model represents a public health leadership approach. This paradigm is characterized by a collaborative style, attitude, and set of knowledge that guides efforts to create international and domestic teams that facilitate biopsychosocial recovery programs.

The U.S. Substance Abuse and Mental Health Services Administration (SAMHSA) and the U.S. Food and Drug Administration (FDA) today launched an initiative to help ensure the safe use of methadone. A prescription drug best known as a treatment for addiction and dependence on heroin and other narcotic pain medicines, methadone is also prescribed to treat moderate-to-severe chronic pain patients. The campaign responds to concerns about an escalating number of poisoning deaths linked to the improper use of this medication.

(Washington, D.C.) - Today, the White House Office of National Drug Control Policy (ONDCP), in partnership with the Substance Abuse and Mental Health Services Administration (SAMHSA), announced the availability of up to $1.2 million for new Drug-Free Communities Support Mentoring Program (DFC Mentoring) grants. An estimated 16 new Mentoring grants will be awarded (approximating $75,000 per grant for up to two years) to drug and alcohol prevention community coalitions representing a cross-section of rural, urban, suburban, and tribal communities. The actual award amount may vary, depending upon the availability of funds and the progress achieved by the awardees.

Findings indicate that most smokeless tobacco users are former or current cigarette smokers

The levels of current smokeless tobacco use increased significantly among adolescent males (aged 12 to 17) from 3.4 percent in 2002 to 4.4 percent in 2007, according to a report based on a series of nationwide surveys. This report shows that the rate of current use of smokeless tobacco (i.e., use within the past month) among the whole population aged 12 and older has remained relatively stable during the same period (in the range of 3.0 to 3.3 percent).

Adults aged 18 to 25 currently using pain relievers for nonmedical reasons increased from 4.1 percent in 2002 to 4.6 percent in 2007, according to a report based on a series of nationwide surveys. However, this report showed encouraging findings among youth aged 12 to 17 in that nonmedical use of pain relievers in the past month had declined from 3.2 percent in 2002 to 2.7 percent in 2007.

(Washington, D.C.)—Today, the White House Office of National Drug Control Policy (ONDCP), and the Substance Abuse and Mental Health Services Administration (SAMHSA) announced the availability of new, Drug Free Communities (DFC) Support Program funds. ONDCP expects to award approximately $17 million—130 new grants—to community drug prevention coalitions throughout the United States. The grant awards are subject to the availability of funds. The deadline to submit DFC grantee applications is Friday, March 20, 2009.

The Substance Abuse and Mental Health Services Administration’s (SAMHSA’s) Center for Substance Abuse Treatment (CSAT) is pleased to announce the availability of the Detoxification and Substance Abuse Treatment Training Manual. This new curriculum, based on Treatment Improvement Protocol (TIP) 45, is for use by clinical supervisors for training staff members about detoxification services for individuals with substance use disorders. The manual includes information on the physiology of withdrawal, pharmacological management of withdrawal, patient placement, and incorporating detoxification services into comprehensive systems of care. It includes step-by-step instructions for providing inservice training.

Adolescents who use drugs at an earlier age are more likely to develop substance dependence or abuse. Preventing or delaying substance use among this population is of utmost importance and has been an ongoing goal of the Federal Government’s Healthy People initiative (http://www.healthypeople.gov/). This issue of The NSDUH Report examines trends in substance use, dependence or abuse, and treatment among adolescents (i.e., youths aged 12 to 17). Data from the 2002 through 2007 National Surveys on Drug Use and Health (NSDUHs) are utilized.

To help, SAMHSA recently cosponsored a second returning veterans behavioral health conference. Substance abuse counselors, mental health treatment providers, advocacy and community-based organizations, and veterans’ families participated in sessions on suicide prevention, jobs, homelessness, understanding traumatic brain injury, and other topics. SAMHSA and the Department of Health and Human Services collaborated with the Departments of Defense and Veterans Affairs to cosponsor the 2008 conference.

National Suicide Prevention Lifeline Network Answers One Millionth Call

SAMHSA convened the Fifth Annual COSIG Grantee Meeting on May 28–30, 2008 at the Hyatt Regency Hotel in Bethesda, MD. Attendees included 170 members of the 19 COSIG States, Federal and Co-Occurring Center for Excellence (COCE) representatives, and invited guests. The meeting focused on the successes of the COSIG program, with special attention given to the first cohort of seven grantees that are in their final year of funding. Key themes were leadership; workforce development; screening, assessment, and treatment planning; peer issues; information systems; and sustaining change.

Underage Alcohol Use: Where Do Young People Drink?

In 2006, more than one in four persons aged 12 to 20 in the United States, or about 10.8 million persons, drank alcohol in the past month (i.e., were current drinkers). Nearly one in five persons aged 18 to 20 drove under the influence of alcohol in the past 12 months in 2006, and each year approximately 1,900 people under the age of 21 die as a result of alcohol-involved motor vehicle crashes. In addition, early initiation of alcohol use is associated with increased likelihood of unprotected sexual intercourse and multiple sex partners.

Cocaine and methamphetamine use among young adults dropped significantly over the past year, while abuse of prescription drugs has risen, according to the Nation’s largest substance use assessment, the 2007 National Survey on Drug Use and Health (NSDUH). The report also showed a spike in drug use among 55- to 59-year-olds, as baby boomers entered that age range.

Inhalant Use and Major Depressive Episode Among Youths Aged 12 to 17: 2004 to 2006

Inhalant use and abuse constitute a public health problem that disproportionately affects youth. The 2006 National Survey on Drug Use and Health (NSDUH) indicated that the prevalence of past-year inhalant use was almost four times higher among persons aged 12 to 17 than among young adults aged 18 to 25 (1.3 vs. 0.4 percent). Among adolescents, inhalant use and abuse have been found to be associated with depressive symptoms. Mental health research focusing on young adolescents who had not previously used inhalants indicates that those with a past-year major depressive episode (MDE) were more than three times as likely as their counterparts with no past-year MDE to initiate use of inhalants.

I was in bed a little before 7 a.m. on Sunday, September 25, 1999, when I received the news that the wreckage of Big Island Air flight 58 had been found in Hawaii Volcanoes National Park. All 10 people on board died when the plane crashed onto the slopes of Mauna Loa, the world’s largest active volcano.

While larger airlines were required to have plans in place under the Aviation Disaster Family Assistance Act of 1996, smaller operators like Big Island Air were not. It had no plan and neither, it turned out, did anyone else.

New Report on Underage Alcohol Use

More than 40 percent of the Nation’s estimated 10.8 million underage current drinkers (persons aged 12 to 20 who drank in the past 30 days) were provided free alcohol by adults 21 or older, according to a nationwide report by the Substance Abuse and Mental Health Services Administration. The study also indicates that 1 in 16 underage drinkers (6.4 percent or 650,000) was given alcoholic beverages by their parents in the past month.

Parents are encouraged to communicate with their children about drugs and alcohol, and some research has examined the extent to which parents are aware of their children’s drug and alcohol use. Studies focusing on the general population have found high levels of parent awareness of adolescent cigarette use, but awareness of alcohol use is low; findings on parent awareness of adolescent marijuana use are mixed.

In 2006, a majority (53.9 percent) of American adolescents and young adults aged 12 to 20 had used an alcoholic beverage at least once in their lifetime. Young people aged 12 to 20 consumed approximately 11.2 percent of the alcoholic drinks consumed in the United States in the past month by persons aged 12 or older. Research shows that underage drinkers tend to consume more alcohol per occasion than those over the legal minimum drinking age of 21.

A new report providing analyses of substance use and mental health patterns occurring in each state reveals that there are wide variations among the states in problems such as illicit drug use and underage drinking, but that no state was immune from these problems. For example, past-month use of alcohol among persons aged 12 to 20 (underage use of alcohol) ranged from a low of 21.5 percent in Utah to a high of 38.3 percent in Vermont. Yet Utah had the highest level of people age 18 or older reporting serious psychological distress in the past year (14.4 percent), while Hawaii had the lowest level (8.8 percent).

Inhalants are substances whose vapors or gas can be sniffed or inhaled to produce mind-altering effects and whose chronic use may cause irreversible damage to the brain, kidneys, and lungs. Found in a range of inexpensive and readily available household, office, industrial, and automotive products, inhalants include substances such as hair spray, shoe polish, glue, gasoline, lighter fluid, spray paints, and other aerosol sprays. Data from the National Survey on Drug Use and Health (NSDUH) have shown that the primary abusers of inhalants are adolescents aged 12 to 17.

In 2006, 2 percent of adolescents aged 12 to 17 (an estimated 510,000 persons) used stimulants nonmedically in the past year, a rate twice as high as that observed among adults aged 26 or older. Across adolescent age groups, the rate of past-year nonmedical stimulant use in 2006 increased from 0.7 percent among youths aged 12 or 13 to 3.3 percent among those aged 16 or 17. Other research has found that stimulant misuse is associated with use of other illicit drugs, criminal justice involvement, and depression.

Methamphetamine and amphetamines are highly addictive central nervous system stimulants. Methamphetamine and amphetamine abuse can lead to serious health consequences, such as rapid or irregular heartbeats, dental problems, mood disturbances, impaired memory, and chronic psychiatric problems. From 1995 to 2005, the percentage of substance abuse treatment admissions for primary abuse of methamphetamine/amphetamine more than doubled from 4 percent to 9 percent.

In the United States, tobacco is used more widely than illicit drugs, and it is implicated in more deaths each year than alcohol and illicit drugs combined. In 2006, 72.9 million Americans aged 12 or older (29.6 percent of the population) were current (i.e., past-month) users of tobacco, and 84.5 percent of these users smoked cigarettes. Nicotine dependence, which is measured for cigarette users, has been found to be associated with increased risk of alcohol and illicit drug use and of mental disorders.

Admissions to substance abuse treatment by State can be monitored with the Treatment Episode Data Set (TEDS), an annual compilation of data on the demographic characteristics and substance abuse problems of those admitted to substance abuse treatment, primarily at facilities that receive some public funding. TEDS records represent admissions rather than individuals, as a person may be admitted to treatment more than once during a single year.

Based on TIP 44: Substance Abuse Treatment for Adults in the Criminal Justice System, the brochure describes what can be gained through substance abuse treatment before trial or while a person is incarcerated, on probation, or on parole. It encourages people to seek help for substance use disorders regardless of their situations.

A new report released today by the Substance Abuse and Mental Health Services Administration's Center for Mental Health Services provides a state-by-state analysis of how Medicaid agencies are exercising their responsibilities for mental health services. Administration of Mental Health Services by Medicaid Agencies combines the results of a series of hour-long telephone interviews conducted in 2005-2006 with State Medicaid directors in all 50 states and the District of Columbia. The report explores how state Medicaid agencies are addressing the organization, funding, policy, management and data issues that arise from increased responsibility for mental health services.
