- Stand-alone Section 8 Excusable non-use form (NOTE: A claim of excusable nonuse
may be included as part of the existing TEAS Section 8 form, following the specific
instructions within that form.)
- Petition form for Post Registration matters
- Section 12(c) Affidavit
- Section 71 Affidavit
- Surrender of registration for cancellation
Petitions or similar filings
- 2.146 petition to the Commissioner
- Petition to Make Special or Request to Make Special

Miscellaneous:
- Substitute forms as a subset of the Response to Office action form, to permit switching
to a "regular" trademark/servicemark, certification mark, collective membership mark, or
collective trademark/servicemark (where simply the wrong form was used initially)
- Stand-alone request to divide form (Note: Prior to a stand-alone form, request to divide functionality, including the required divisional fee, will be added to the SOU form.
Currently scheduled for January 2009.)
- Withdrawal of Attorney form, where one individual is withdrawing, but firm retains
representation (so that correspondence address does not change to that of applicant)
