Confidential Financial Disclosure Report (OGE Form 450)

PLEASE READ THE FOLLOWING 4 POINTS BEFORE ACCESSING OR DOWNLOADING FINANCIAL DISCLOSURE REPORT FORMS.

  1. IMPORTANT: Please do not send your completed OGE Form 450 to OGE. Your agency, not OGE, is responsible for reviewing and maintaining the forms for its employees. Contact your agency ethics official to obtain the correct address for mailing your form.

  2. It is critical that you read through the entire form, gather your financial records that contain the information requested by the form, and then fill out the form.

  3. Filers must print and submit paper forms with handwritten signatures to their agency ethics officials unless the agency has approved the use of the form's digital signature capability. Filers should check with their agency ethics official to find out whether their agency accepts a digitally signed confidential financial disclosure form. Agencies interested in using the form's digital signature's capabilities must comply with OGE DAEOgram DO-09-004 (PDF-HTML-TXT).

  4. The reporting threshold for gifts and travel reimbursements received from one source (Part V) was further increased, effective as of January 1, 2008, to more than $335, with a $134 or less de minimis aggregation exception (see OGE DAEOgram DO-08-010 (PDF-HTML-TXT) of March 25, 2008 for more details on this change). OGE has revised the OGE Form 450 to reflect this change.

How to Download the OGE Form 450 (2008 edition)

The revised edition of the OGE Form 450 requires the use of Version 8 or 9.1 of the free Adobe Reader. If you have a lower version of Adobe Reader on your computer, you must first uninstall that version. Although the form will work in Version 8, OGE recommends that you install Version 9.1.

A. To uninstall an older version of the reader:

  1. Left-click on the Start button.
  2. Left-click on Control Panel (Windows XP) or click on Settings then Control Panel (Windows 2000).
  3. Double-click on Add or Remove Programs. If you do not have administrator rights, you will not be able to remove the          program. Contact your agency’s IT staff for assistance.
  4. When a new window opens, select the Adobe Reader from the list of programs and click Remove.
  5. Adobe Reader has now been removed from your computer.

B. To install Adobe Reader 9.1, go to www.adobe.com, and download the free reader. Also download and install any updates to the reader as they become available.

C. To download the OGE Form 450 to your computer:

  1. Right-click on the hyperlink to the form (see hyperlink below). Most filers should click on the first hyperlink, the version that automatically adds continuation pages.
  2. Left-click on Save Target As. (Depending upon the browser you are using, this may appear as Save Link As or Save Linked Content As.)
  3. A Save As window will prompt you to save the form to a directory on your computer.
  4. Select a directory and give the form a unique name such as “ogeform450_johnsmith.pdf.” Click the Save button.
  5. Open the form from your directory.
  6. Fill out the form and save your changes. CAUTION: When you save the data you enter on the form, do not use the option named Save A Copy. This option will prevent you from making any further changes to the form.

STOP! Do you still have Adobe Reader 7 or lower on your computer? If you do, you will not be able to update the information that you type into the OGE From 450. Go back to the "How to" section and follow the instructions on how to uninstall the old version of the reader and how to install Version 9.1.

  1. OGE Form 450 — PDF Version
    You can automatically add continuation pages to the form in this version. In order for the form to work properly, add a continuation page for a Part from the last page of the Part rather than from any page in the Part.

  2. OGE Form 450Section 508 Version (only for use with screen reader software)
    This version meets the accessibility requirements of Section 508 of the Rehabilitation Act Amendments of 1998 (Section 508). Due to constraints of screen reader software, some automated features, such as automatic generation of continuation pages, are not available in the 508 Compliant version of the form. Instead, we have created three separate files (listed below) that represent the continuation pages for Part I, Parts II and III, and Parts IV and V. Filers can save as many copies as necessary of these files.
