How do I report a death?

Answer:

If you are filing an Option-C claim, or if you die and you are an active employee, your survivor should contact your Human Resources Office to report the death. After you retire, you or your survivor should contact OPM’s Retirement Operations Center at 1-888-767-6738 or send an email to retire@opm.gov.
