Information regarding Form 1099-G Tax Withholding

 

Form 1099-G will be mailed to all who received unemployment insurance benefits at the end of January of each year. The form will include the amount of benefits paid and other information to meet federal, State, and personal income tax needs for the tax year.

To ensure timely receipt of Form 1099-G, make sure that the Unemployment Insurance (UI) Division has your most current mailing address. To update your address or to obtain a duplicate Form 1099-G if the original was not received within a reasonable period of time, contact your local unemployment insurance claims office. Click on the “Contact Us” link on the UI homepage to obtain local office phone numbers.
