This FOM Internet Application (FOMIA) is for use by NCUA member credit unions only,
not the general public. This system can be used for adding associational and/or
occupational groups of 2,999 potential members or less as well as the non-natural
person corporate account associated with that group. Once the group(s) is approved,
and after the Board Resolution is adopted, you may begin serving members of the

NCUA member credit unions wishing to use this system must first acquire a PIN. You
can obtain your PIN by contacting your NCUA Regional Office's Division of Insurance.

NEW SYSTEM REQUIREMENTS:
