Getting Started

If you are receiving federal benefit payments, and wish to receive these payments by direct deposit, please follow these directions:

Complete one of the direct deposit sign-up forms:

  • For social security, supplemental security income, civil non-military federal retirement and railroad retirement payments use FMS Form 1200pdf | (Spanish)pdf
    (Mail to the address at the bottom of the form.)

  • For all other non-vendor federal payments, use SF 1199Apdf
    (Mail to the appropriate paying agency, or contact your financial institution.)

  • For payments to federal vendors, use SF 3881PDF File
    (Mail to the appropriate paying agency, or contact your financial institution.)
