Summary: This project will develop a health information exchange with the Collaborative (i.e., businesses, health systems and plans, philanthropic organizations, government, schools, and the University of New Mexico) to improve care coordination and chronic disease outcomes, while reducing unnecessary costs of care. The project design includes development of an organizational infrastructure and disease management prototypes (diabetes, pediatric asthma, depression, and low back pain) utilizing e-health strategies to improve care and reduce costs. It also will establish a rural pilot program to serve as a model for other rural areas, and will evaluate development, implementation, and outcomes of the health information collaborative. The study population includes women, children, and minority groups.


*Project includes children or children's health care issues but does not focus exclusively on children.
