Are you looking for a doctor, health center or organization that can help you with your health care needs and questions? HealthFinder.gov can help you find a doctor or other health care provider, health center, home health care, hospital and much more. MedlinePlus can also help connect you to doctors, hospitals and clinics that specialize in specific conditions or disabilities.

The Centers for Medicare & Medicaid Services (CMS) has a tool that can help you find information on how well hospitals care for patients. Hospital Compare also provides results from a survey of patients about the quality of care they received during a recent hospital stay.

Nursing Home Compare has information about every Medicare and Medicaid-certified nursing home in the country. Before you get started, you should look into other long-term care choices like community-based services, home care or assisted living.

Early & Periodic Screening, Diagnostic & Treatment (EPSDT) is Medicaid's comprehensive and preventive child health program for individuals under the age of 21. This program includes general health screening and vision, dental and hearing services. Contact your state's Medicaid agency for more information.

Health Care in Your Community

The Health Resources & Services Administration (HRSA) has health centers in most cities and many rural areas. These centers offer primary care even if you have no health insurance. You pay what you can afford, based on your income. HRSA also funds a nationwide network of clinics and health centers for people living with HIV/AIDS.

Go Local, a service of the National Library of Medicine and the National Institutes of Health, provides state and local resources for health-related services. This Web site can help you find doctors, dentists and other health care professionals and facilities in your state.

State Offices of Rural Health work to ensure that people living in small rural communities have access to medical care and prevention services.

The Indian Health Service (IHS) is the federal government's health service for American Indians and Alaska Natives. IHS is responsible for providing preventive and community health care in hospitals and clinics throughout the United States.

Maternal & Child Health Bureaus take care of the physical, psychological and social needs of mothers and children.

The Centers for Mental Health Services (CMHS) can connect you to counseling and mental health services in your state.

Long-Term Care

The National Clearinghouse for Long-Term Care Information has information and resources that can help you and your family plan for future long-term care (LTC) needs. LTC is important because Medicare and private health insurance programs do not pay for the majority of long-term care services that most people need, such as help with personal care and activities including dressing or using the bathroom.

Aging & Disability Resource Centers (ADRCs) are the single points of entry into the long-term care system for older adults and people with disabilities. Sometimes called "one stop shops" or "no wrong door" systems, single entry points address many of the frustrations consumers and their families experience when trying to find needed information, services and supports.

The Eldercare Locator can help you find local agencies that provide older persons and their families with home and community-based services like transportation, meals, home care and caregiver support services.
