TO FULFILL THIS REQUIREMENT, TRICARE MANAGEMENT ACTIVITY WILL INITIATE A NEW COMPREHENSIVE DENTAL HEALTH CARE DELIVERY SYSTEM KNOWN AS THE ACTIVE DUTY DENTAL PROGRAM (ADDP) ON 01 AUGUST 2009. DENTAL COVERAGE FOR ADSMS REFERRED FROM A USCG OR DOD DENTAL TREATMENT FACILITY (DTF) FOR CIVILIAN DENTAL CARE, AND B. DENTAL COVERAGE FOR ADSMS SERVING IN A REMOTE AREA. 3. YOU ARE ELIGIBLE FOR THE ADDP IF YOU MEET THE FOLLOWING: A. ADSMS REFERRED BY USCG OR DOD DTF FOR SPECIALTY DENTAL CARE. B. REMOTE ADSMS WHO LIVE AND WORK MORE THAN 50 MILES FROM A USCG OR DOD DTF. C. RESERVIST ON ACTIVE DUTY ORDERS 31 DAYS OR MORE WHO MEET EITHER CRITERIA IN PARAGRAPH 3A OR 3B. 4. THE DEFENSE ELIGIBILITY ENROLLMENT REPORTING SYSTEM (DEERS) IS THE DATABASE OF RECORD FOR ELIGIBILITY. SERVICE MEMBERS SHOULD ENSURE THAT DEERS ACCURATELY DISPLAYS THEIR MEDICAL/DENTAL ELIGIBILITY BY CALLING 1-800-538-9552 TO VERIFY OR UPDATE YOUR STATUS ANNUALLY. SPONSORS MAY ALSO VERIFY OR UPDATE THEIR INFORMATION ONLINE AT HTTPS://WWW.DMDC.OSD.MIL/MYDODBENEFITS (ADDRESS MUST BE TYPED IN LOWER CASE). 5. UNITED CONCORDIA (UCCI) WILL SEND AN INFORMATIONAL PACKAGE TO REMOTE ELIGIBLE SERVICE MEMBERS IN JUNE OF 2009 BASED ON THE ADDRESS LISTED IN DEERS. BEGINNING 01 AUG 09, ADSMS WHO SEEK DENTAL CARE OUTSIDE A USCG OR DOD CLINIC NEED TO CONTACT UCCI TO OBTAIN AN APPOINTMENT CONTROL NUMBER. 6. THE CONTRACT WILL ONLY PAY FOR COVERED DENTAL BENEFITS AND THOSE DENTAL BENEFITS THAT REQUIRE AND HAVE RECEIVED PREAUTHORIZATION. UCCI MAY ASSIST IN MAKING AN APPOINTMENT. INITIATING DENTAL CARE REQUIRING AUTHORIZATION WITHOUT WRITTEN AUTHORIZATION FROM UCCI MAY RESULT IN THE SERVICE MEMBER BEING RESPONSIBLE FOR PART OR ALL OF THE COST OF TREATMENT. FAMILY COVERAGE MAY CONTINUE AT A REDUCED MONTHLY PREMIUM RATE.
