ALCOAST 118/09 COMDTNOTE 6120 SUBJ: PERIODIC HEALTH ASSESSMENT (PHA) FULL IMPLEMENTATION A. COAST GUARD PERIODIC HEALTH ASSESSMENT, COMDTINST 6150.3(SERIES) 1. ALL MEMBERS WHO RECEIVE THEIR CARE AT A COAST GUARD CLINIC ARE REQUIRED TO HAVE THEIR ANNUAL PHA. ALL CG CLINICS, INCLUDING PREVIOUSLY DESIGNATED PILOT TEST SITES (D9 AND D17), ARE NOW PERFORMING THE YEARLY REQUIRED PHA. 2. BEGINNING 1 MARCH 2009, THE PHA WILL BE REQUIRED FOR WHITE SPACE PERSONNEL. WHITE SPACE PERSONNEL ARE DEFINED AS ALL COAST GUARD ACTIVE DUTY (AD) AND SELECTED RESERVISTS (SELRES) NOT ON AD ORDERS 31 DAYS OR MORE, WHO DO NOT RECEIVE PRIMARY CARE SERVICES AT A COAST GUARD CLINIC. ALTHOUGH TITLED RESERVE HEALTH READINESS PROGRAM (RHRP), THIS PROGRAM MEETS THE SERVICE NEEDS FOR WHITE SPACE ACTIVE AND SELRES COMPONENTS. 3. WHITE SPACE AD AND SELRES MEMBERS MUST CALL 1-888-MYPHA99 (1-888-697-4299) TO MAKE A PHA APPOINTMENT USING THE RHRP CALL CENTER DURING THEIR BIRTH MONTH PERIOD. A BIRTH MONTH PERIOD IS DEFINED AS THE ACTUAL MONTH OF BIRTH AND THE PRECEDING TWO MONTHS. DO NOT USE THE PREVIOUSLY PUBLISHED PHONE NUMBER IN REF A. WHEN A MEMBER CALLS THEY ARE ENCOURAGED TO HAVE SEVERAL DATES AND TIMES WHEN THEY WILL BE AVAILABLE FOR A PHA APPOINTMENT. A PHA KIT WILL BE SENT BY RHRP TO THE MEMBER BEFORE THE PHA APPOINTMENT. 4. IF THE RHRP CALL CENTER CANNOT FIND A CLINICAL PROVIDER WITHIN 50 MILES OF THE MEMBERS HOME OR UNIT, THE MEMBER WILL BE INFORMED THAT HE OR SHE WILL BE CONTACTED WHEN A PARTICIPATING CLINICAL PROVIDER IS LOCATED. 5. ALTHOUGH THE PHA HAS REPLACED THE ROUTINE 5-YEAR PHYSICAL EXAMINATION, IT IS NOT AN ANNUAL COMPREHENSIVE PHYSICAL EXAMINATION. THE PHA IS A PREVENTIVE HEALTH SCREENING. PER REF A, ALL MEMBERS MUST BRING THEIR COMPLETED FLEET HEALTH RISK ASSESSMENT (HRA) AND, IF PRESCRIBED, TWO PAIRS OF GLASSES. WHITE SPACE MEMBERS SHOULD ALSO BRING THEIR PHA KIT, BUT SHOULD NOT BRING THEIR HEALTH RECORD. AT A MINIMUM, A MEMBER SHOULD EXPECT THE FOLLOWING: A. REVIEW OF THEIR FLEET HRA AND DISCUSSION OF THE MEMBERS GENERAL HEALTH STATUS AND HEALTH RISK BEHAVIORS (E.G. SMOKING, LACK OF EXERCISE, ETC.). B. BLOOD PRESSURE CHECK. C. VISION SCREENING. D. REVIEW OF ANY MEMBER DIVULGED HEALTH CONCERNS AND ASSESSMENT OF EXISTING CONDITIONS. E. ADDITIONAL SERVICES MAY INCLUDE UPDATING A MEMBERS MEDICAL READINESS (E.G. HIV TEST, IMMUNIZATIONS, ETC.), PERFORMING AGE AND GENDER SPECIFIC CLINICAL PREVENTIVE SERVICES REQUIREMENTS (E.G. CHOLESTEROL TEST, PAP SMEAR, ETC.). FOR WHITE SPACE PERSONNEL, HIV TESTING WILL BE PERFORMED BY EXISTING LOCAL COAST GUARD PROCESSES. F. IF A MEMBER IS HEALTHY, AND HAS NO READINESS OR CLINICAL PREVENTIVE SERVICE NEEDS, THEIR PHA VISIT MAY BE BRIEF. G. WHITE SPACE AD AND SELRES MEMBERS WHO RECEIVE THE PHA VIA THE RHRP CONTRACTORS WILL BE REFERRED BACK TO THEIR PRIMARY CARE MANAGER (PCM), EITHER DOD OR CIVILIAN, FOR ANY NECESSARY FOLLOW-UP. ALL FOLLOW-UP DOCUMENTATION AND RESULTS SHALL BE RETURNED TO THE MEMBERS HEALTH RECORD CUSTODIAN BY THE RHRP CONTRACTOR. 6. ONCE A MEMBER MAKES A PHA APPOINTMENT VIA THE TOLL-FREE RHRP CALL CENTER, THAT IS THE MEMBERS APPOINTED PLACE OF DUTY FOR THAT TIME PERIOD AND THEY MUST SHOW UP FOR THAT APPOINTMENT. THE COAST GUARD INCURS A SIGNIFICANT COST (UNDER THE CONTRACT) FOR NO SHOWS. MEMBERS WHO ARE MARKED AS A NO-SHOW WILL BE NOTIFIED BY THE HEALTH, SAFETY, AND WORK-LIFE SUPPORT ACTIVITY (HSWL SUPACT), AS WELL AS THEIR CHAIN OF COMMAND. IF A MEMBER MUST CANCEL AN APPOINTMENT, THE MEMBER MUST CALL 1-888-MYPHA99 (1-888-697-4299) NO LATER THAN 72 HOURS PRIOR TO THE APPOINTMENT. 7. MEMBERS USING THE RHRP CALL CENTER SHOULD NOT NEGOTIATE WITH THE CONTRACTORS REGARDING THE PROVIDERS ASSIGNED TO THEM. THE RHRP PROVIDER WILL MOST LIKELY NOT BE THE MEMBERS CIVILIAN OR MILITARY PRIMARY CARE PROVIDER. 8. SELRES MEMBERS ARE AUTHORIZED AND SHOULD REQUEST ONE READINESS MANAGEMENT PERIOD IN DIRECT ACCESS FOR THE DATE OF THE PHA - THIS CONSTITUTES OFFICIAL ORDERS FOR THE PHA. 9. MEMBERS WHO HAVE COMPLETED THE PHA PROCESS CAN COMPLETE AN ANONYMOUS PHA SATISFACTION SURVEY LOCATED AT HTTP://WEBAPPS.MLCA.USCG.MIL/DYNAMICSURVEY. CLICK ON "TAKE A SURVEY".
