THIS ALCOAST CLARIFIES THE PHA PROCESS AND EXPECTATIONS FOR SELRES MEMBERS IN A WEEKEND DRILLING STATUS OR ON ACTIVE DUTY ORDERS FOR 30 DAYS OR LESS. 2. HOME UNIT OPFAC NUMBER (ALSO KNOWN AS UNIT IDENTIFICATION CODE (UIC)). B. HEALTH RECORD CUSTODIAN (HRC) CONTACT INFORMATION: OPFAC NUMBER, NAME, WORK ADDRESS, AND WORK PHONE NUMBER. 4. WHEN A SELRES MEMBER CALLS TO SCHEDULE AN RHRP PHA APPOINTMENT: A. THE EXPECTATION IS THAT A CONTRACT HEALTH CARE PROVIDER WILL BE FOUND WITHIN 50 MILES OF THE MEMBERS HOME OR UNIT ADDRESS. IF A HEALTH CARE PROVIDER IS AVAILABLE WITHIN 50 MILES, THE SERVICE MEMBER WILL BE SCHEDULED FOR A PHA APPOINTMENT AT THAT TIME. B. IF THERE IS NO CONTRACT HEALTH CARE PROVIDER WITHIN 50 MILES OF EITHER LOCATION, THE MEMBER WILL BE PLACED IN A QUEUE AND WILL BE CONTACTED BY THE CALL CENTER WHEN AN APPOINTMENT IS FOUND WITHIN 50 MILES. OF NOTE, THE 50 MILE DISTANCE IS DETERMINED BY USING MAPPOINT. 5. MEMBERS MUST BRING THE FOLLOWING ITEMS TO THE APPOINTMENT: A. COMPLETED AND PRINTED FLEET HEALTH RISK ASSESSMENT(HRA), AVAILABLE AT HTTP://164.167.141.36/PLS/NEWHRA/HRA. WEB ADDRESS MUST BE ENTERED IN LOWERCASE. REMEMBER TO ENTER AN "R" BEFORE THE UNIT OPFAC (ALSO KNOWN AS UIC) AS LOGIN IDENTIFICATION IF YOU ARE A SELRES MEMBER. B. PHA KIT (UNIQUE FOR EACH MEMBER AND MAILED TO MEMBER AFTER APPOINTMENT PHONE CALL). C. TWO PAIR OF EYEGLASSES (IF APPLICABLE). D. ANY NEW MEDICAL DOCUMENTATION (CIVILIAN OR MILITARY) THAT RELATES TO HIS OR HER MEDICAL READINESS (E.G. IMMUNIZATIONS GIVEN BY MEMBERS CIVILIAN HEALTH CARE PROVIDER THAT ARE NOT REFLECTED IN CGBI, ETC). 6. EACH PHA KIT WILL HAVE AN INVENTORY LIST (THE DOCUMENT IS LABELED "SHIP KIT PICK LIST"). MEMBERS MUST REVIEW THE CHECKLIST AND VERIFY THAT THE CONTENTS IN THE KIT MATCH THE CHECKLIST. IF THERE IS A MISSING ITEM, THE MEMBER MUST CONTACT THE RHRP PHA CALL CENTER (1-888-697-4299) TO INFORM THEM OF THE MISSING ITEMS. 7. MEMBER IS NOT TO BRING HEALTH RECORD TO THE APPOINTMENT. 8. IF THE RHRP HEALTH CARE PROVIDER DETERMINES THAT FOLLOW-UP CARE IS NEEDED, THE MEMBER WILL BE INFORMED IN WRITING OF THE SPECIFIC ITEMS NEEDING FOLLOW-UP. THE MEMBER MUST MAKE AN APPOINTMENT WITH HIS OR HER CIVILIAN HEALTH CARE PROVIDER. (THE RHRP HEALTH CARE PROVIDER WILL NOT PROVIDE FOLLOW-UP CARE.) MEMBER IS THEN EXPECTED TO COMPLETE THE FOLLOW-UP AND SUBMIT REQUIRED DOCUMENTATION TO HIS/HER HRC WITHIN 30-60 DAYS OF THE PHA. IN ADDITION, RHRP WILL FORWARD ALL PHA MEDICAL NOTES AND PAPERWORK TO THE HRC. IF A DISQUALIFYING CONDITION IS NOTED THE MEMBERS CONDITION WILL BE FURTHER REVIEWED TO CONSIDER THE NEED OF CONVENING A MEDICAL EVALUATION BOARD (MEB). 9. SELRES MEMBERS ARE TO MAINTAIN FITNESS AND READINESS 24/7. AS SUCH, MEMBERS ARE REMINDED THAT ONLY THE SCREENING TESTS REQUIRED FOR THE PHA ARE PROVIDED BY THE CG. ANY FOLLOW-UP TESTING TO PROVE FITNESS IS THE RESPONSIBILITY OF THE MEMBER, AND MUST BE PROVIDED IN A TIMELY FASHION AS NOTED ABOVE. 10.
