Native American Title VI Grants - Standard Form 269 (SF-269)

Title VI Grants require annual Financial Status Reports (SF-269) be submitted for each year within the project period. The report can be completed using Title VI online Reporting Tool. Hardcopy report can be mailed to:

U.S. Department of Health and Human Services
Administration on Aging
Attn: Office of Grants Management
Washington, DC 20201

Requests to carry over Title VI grant funds from one budget period to the next must be submitted to Office of Grants Management (same as above) on an annual basis.

  • OMB Standard Form (SF) 269

If the unobligated funds as shown of the SF-269 have already been drawn down from the Payment Management System of HHS’ PSC, they must be returned to AoA. Use of unobligated funds after the grant’s budget period has expired without an approved carry over request will result in an audit disallowance.
