PB 1 and 2 Waiver Request Form (59kb) About PDF Files The burner shall fill out this waiver request form completely if requesting a waiver from a requirement that can be waived by the Department.Available in MS Word 242kb.

Smoke Management PB-I R1 (69kb) About PDF Files This registration form shall be filled out completely by any users of prescribed fire that intend to burn or do burn more than ten (10) acres, or more than 1,000 cubic feet but less than 5,000 cubic feet of pile volume of vegetative material per day or emits less than one ton of PM10 emissions per day. Smoke Tracking Form (141kb) About PDF Files This form is to be completed the day of the burn and shall be submitted to the Department within two weeks following completion of burn project. For PB-I and PB-II burns. Available in MS Word 175kb.

Smoke Management PB-II ERT Form (72kb) About PDF Files The burner shall implement at least one Emission Reduction Technique (ERT)to be used in conjunction with fire and do not replace fire. Available in MS Word 217kb.

Smoke Management PB-II R1 Registration (72kb) About PDF Files This registration form shall be filled out completely by any users of prescribed fire that intend to burn or do burn more than ten (10) acres, or more than 1,000 cubic feet, but greater than 5,000 cubic feet of pile volume of vegetative material per day or emits one (1) ton or more of PM10 emissions per day. Part 21 Conditional Open Burn Form (55kb) About PDF Files This form shall be filled out completely to provide notification to the Department for open burns, and shall comply with local fire and safety codes. Available in MS Word 216kb.

Part 21 Single and Multiple Event Open Burn Form (59kb) About PDF Files This form shall be filled out completely by a person seeking a multiple or single event open burn permit.Available in MS Word 233kb.

Ventilation Category (MSExcel) (53kb) Use this form to document the ventilation category for the burn for all burn days. This form will be used for informational purposes to determine the number of times the ventilation was good or better and how often the waiver needed to be implemented.

Visual Monitoring (MSExcel) (41kb) Use this form to document the visual monitoring for the burn for poor ventilation category burn days. It will meet your requirements for documenting visual monitoring on the day of the burn.
