Formula Grants - Standard Form 269 (SF-269)

AoA’s formula grants have a one year project period however an additional two years to expend these funds is allowed because of the hierarchical structure of the aging network’s funds distribution system.

A Financial Status Report (SF-269) is due semi-annually. Reports are due within 30 days of the end of the 2nd and 4th quarters (April 30 and October 31).

In addition, a Financial Status Report AoA Supplemental Form to SF-269 for the Title III program is due semi-annually.

With semi-annual financial reporting for each of the three years that the grant remains active, and a final report due 30 days after the project end date, a total of six financial status reports are required for each formula grant.

  • OMB Standard Form (SF) 269

This report forms the basis of the close out of the grant. Any project funds that remain unused at the end of the budget period as reported on the final SF-269 are returned to the Treasury, unless the grantee has requested an extension of the time available to expend the funds during the time the grant was active.
