Acknowledgments

An undertaking of this magnitude could not have been accomplished without the involvement of many people, both inside and outside of the Agency for Healthcare Research and Quality. It truly has been a collaborative effort.

Foremost, a note of special gratitude is extended to the vast number of patient safety researchers who willingly shared with us the various stages of progress of their own research and made this four-volume set possible. We believe their work collectively makes a significant contribution to the field. Just as numerous were a large collection of peer reviewers given the task of ensuring the quality and appropriateness of the submitted manuscripts. We appreciate their effort. A list of the peer reviewers is in each volume. Brent James, Paul Schyve, Lucian Leape, and Mary Wakefield deserve our thanks for "adopting" individual volumes, reading their contents, and helping to set the stage for each volume with their written prologues. Also very much in evidence were the organizational skills of Suzanne Streagle, Dan Sangria, and their colleagues at Westat who kept track of the submitted abstracts and manuscripts, and maintained communication with the authors and reviewers.

We would like to thank our Department of Defense partners for their support and involvement throughout the publishing process.
