Most Common Motor Vehicle Report Submission Errors

Dear WESS Customer,

The Naval Safety Center thanks you for your mishap reports. NSC has received valuable motor vehicle mishap information from WESS users. In order to improve the quality of submissions, this message is intended to inform the users of the most common WESS motor vehicle report submission errors.

  1. The most common errors are: Incorrect Mishap Data Entry Selection. (This is the first screen after user selects 5102 mishap) For Private Motor Vehicle Mishaps select “Motor/Recreational Vehicle (Private)”.

  2. Users occasionally do not include all vehicles involved in the incident. In order to perform comprehensive analysis, all involved vehicles must be included. For motor vehicle mishaps, vehicles are entered in the “Involved Property” section.

  3. Users fail to include all people involved in the mishap. Please include all individuals who were occupants in the involved vehicles and any one else who had a direct effect on the mishap. Individuals may be added via the “Involved People” section.

  4. Important mishap information is occasionally omitted. Most frequent omissions are:

    1. Year/Make/Model of involved vehicles.
    2. Personal Protective Equipment. In the Personal Protective Equipment screen for motorcycle mishaps, the user should always select clothing, gloves, helmet, and boots. The user will then be prompted by the application whether the PPE was required, and whether the PPE was used and functioned properly. For 4-wheel mishaps, the user should always select “Vehicle Safety". At a minimum, the user should provide amplifying information about seat belt usage under “Vehicle Safety”.
    3. Driver training for motorcycles and other Government/Private motor vehicle mishaps.
    4. Mishap Cause Code and Cause Code Narrative.
    5. Incomplete final narratives. Please provide a complete picture of the mishap in the final narrative section. The user is not limited in the amount of text space in the final narrative block. Please do not include personal identifiers such as names, dates, commands, and vehicle types.
