The Role of Activity Safety Managers in Class "A" Mishap Investigations

• Secure the mishap scene and equipment involved

• Report the mishap through the chain of command and the Naval Safety Center

• Contact the mishap investigation hotline at (757) 444-3520 ext. 7145 (DSN 564)

• Coordinate photographs of scene

• Complete initial witness list for team

• Note environmental conditions

• Provide mishap investigation team with points of contact list

• Provide logistical/administrative support to the mishap investigation team
