Forms

You can download or order forms online or order them by calling 800-722-2678. Some of the forms used most often are:

  • New Account Request (PDF 5182) opens an account.
  • Treasury Marketable Securities Tender (PDF 5381) places an order to buy a security.
  • Transaction Request (PDF 5178) changes a bank account, address, name on account and more.
  • Sell Direct Request (PDF 5179-1) authorizes the sale of your security before it matures.
  • Security Transfer Request (PDF 5179) transfers securities out of TreasuryDirect or between accounts.

Unless your form states otherwise, mail the form and any requested documentation to your Treasury Retail Securities Site.

Mail Notifications

You will receive periodic communication from us by mail. The information you will get includes:

  • Tax Forms that report your annual earnings.
  • Reinvest Direct Notices tell you when your securities will mature and how you can reinvest.
  • Statements of Account provide details on your holdings. You can request a Statement of Account online or by phone.
