In order to better serve you, this form will allow you to go to a printable page that should be mailed to the office for processing.
If you are a constituent experiencing problems in dealing with a federal agency, complete and mail the following form to our New York office so that we may look into the matter. All correspondences and phone calls pertaining to casework should be directed to the district office and not our Washington office.
Make sure to include all pertinent information, including claim numbers. For example, a matter involving Social Security requires your Social Security number; if you're having veterans' benefits issues, we will need your VA claim number; problems with the Internal Revenue Service requires a taxpayer identification number. Please copy and send any related documents or correspondences that you have received from the agency in question.
Please Note: The Privacy Act of 1974 requires that Members of Congress or their staff have written authorization before they can obtain information about an individual's case. We must have your signature to proceed with a casework inquiry.
