AHRQ has prepared an Informed Consent and Authorization Toolkit to help
health services researchers with just this issue. It should be published
this summer. It not only provides a sample of a health literacy-friendly
informed consent and authorization form, but also addresses the process
of the informed consent discussion (e.g., privacy, interpreters,
confirming understanding). It's in the final stages of being tested, but
if you have any urgent need I may be able to send you a pre-publication
copy.

Also, AAMC is trying to develop health literate informed consent
template for clinical research. The contact person there is Irena
Tartakovsky [itartakovsky at aamc.org].
