You are the heart of your family... take care of it. (Usted es el corazon de la familia... cuide su corazon)

This bilingual (Spanish and English) booklet explains the link between diabetes and heart disease for Hispanics/ Latinos. It encourages patients to work with their health care team to manage their blood glucose, blood pressure, and cholesterol. A wallet card allows patients to track their target numbers.

Este es un folleto bilingüe (español e inglés) fácil de leer, y explica la conexión entre diabetes y enfermedad cardiaca en los hispanos y latinoamericanos. Este folleto ilustrado anima a los pacientes a que den pasos para controlar no sólo sus niveles de glucosa (azúcar) en la sangre, sino también su presión arterial y el colesterol. El folleto incluye una tarjeta que puede extraer y colocar en su billetera para hacer el seguimiento de estos números como meta.
