NDI Matching Criteria
The NDI program will permit a match to be listed if any 1 of 7 conditions are satisfied.

NDI Application Form

NDI User Fees
Fees are based on the number of records submitted. The fee schedule includes a worksheet for calculating your total NDI charge.
NDI User's Guide
Contains the first two chapters of the User's Guide. These chapters explain the steps in the NDI process and how to submit your records for an NDI search. If you would like to receive an e-mail attachment containing a draft of the entire User's Guide, please send an email ndi@cdc.gov to request the draft document.
