I Welcome the Opportunity to Assist My Constituents
 
If you are a constituent in my district and you have a problem or concern surrounding a claim with a federal agency (such as the Social Security Administration, the Department of Veterans Affairs, any military branch, the Internal Revenue Service and the United States Citizenship and Immigration Services), my office would be happy to assist you.
 
Unfortunately, I can not assist those who are not a resident of New York's 19th Congressional District.  Please contact your own Representative for assistance. CLICK HERE to find your Representative.
 
What You Need to Do
 
To begin, please fill out a case questionnaire and privacy form.  Due to the Privacy Act of 1974, your written request for assistance is required.  For your convenience, these forms are available online or at any of my offices.

Please print and fill out the form that most closely corresponds to your case and mail it, with a privacy form, to the office closest to you.
Questionnaires
Please note: I am prohibited from intervening, in any way, in judicial matters, pardon or parole cases.  I also cannot authorize the release of prisoners.  In addition, I do not have jurisdiction over prison transfer cases but as a courtesy to the inmate or family member, I will forward those requests to the Department of Corrections or Federal Bureau of Prisons.
