FAR 52.203–13 Reporting Requirement

To file a mandatory disclosure as required by the FAR clause, please click the report link below to notify in writing, the VA OIG whenever the contractor has evidence that a principal, employee, agent, or subcontractor of the Contractor has committed a violation of the Civil False Claims Act or a violation of Federal criminal law involving fraud, conflict of interest, bribery, or gratuity violations in connection with the award, performance, or closeout of a contract or any related subcontract.

The individual making the report must be an officer or manager empowered to speak for the company when filing this report. If you wish to provide information that does not fall within these guidelines, please visit the VA Inspector General Hotline website.

Instructions

You will need the following information to complete your report:

  1. Your full name and title.
  2. State whether you are a principal officer or manager empowered to speak for the company.
  3. Provide us the primary point of contact (name, title, and telephone number) for this report.
  4. Provide your company’s name, address, and telephone number.
  5. State whether you are the Prime Contractor or a Sub Contractor.
  6. Provide the Contract Number and, if applicable, the number(s) of all underlying task orders, blanket purchase agreements, purchase orders, or other related procurement documents relating to VA.
  7. If the contract was awarded by another agency, identify the agency and provide that agency’s contracting officer’s name and telephone number.
  8. For each VA awarded contract, task order, purchase order, or blanket purchase agreement, identify the VA contracting officer or purchasing agent by name and facility.
  9. Identify the products and/or services the contract provided.
  10. Explain whether the conduct involves criminal activity, civil fraud, and/or overpayments.

Please include a detailed description of the facts and circumstances surrounding the reported activities, including the evidence forming the basis of this report, the names of the individuals involved, dates, location, how the matter was discovered, potential witness(es) and their involvement, and any corrective action taken by the company.
