GRANTS
The Office of Minority Health (OMH) administers grant programs in an effort to facilitate community linkages and strategies that use scarce resources efficiently across national lines. These grants were awarded to support community organizations and science-based efforts to eliminate health disparities. The following grants have been awarded and are, at this time, not open to receive applications.
2009
- New OMH Funds Target HIV/AIDS Outcomes for Ex-Offenders
Approximately $2 million is available for 8 to 10 new, competitive, cooperative agreements under the HIV/AIDS Health Improvement for Re-entering Ex-Offenders Initiative.
Deadline: May 22, 2009.
2008
- OMH Invites Applications for HIV/AIDS Capacity Development Grants
There is $3 million available for OMH competitive grants as part of the Collaborative Technical Assistance and Capacity Development Demonstration Grant Program for the Coordination and Delivery of a Continuum of HIV/AIDS-Related Services in Minority Communities (CTA/CD). Twelve to 15 grant awards are expected.
2007
- OMH Invites Applications for AI/AN Health Disparities Program Grants
Approximately $1 million is available for new, competitive grants under OMH's American Indian/Alaska Native Health Disparities Program.
- OMH Invites Applications for Bilingual/Bicultural Demonstration Grants
Approximately $2.3 million is available for new, competitive grants under OMH's Bilingual/Bicultural Demonstration Grant Program ("Bilingual/Bicultural Program").
- OMH Invites Applications for HIV/AIDS Health Promotion and Education Grants
Approximately $2.3 million is available for new, competitive grants under OMH's HIV/AIDS Health Promotion and Education Program ("HIV/AIDS Program").

2006
- Youth Empowerment Demonstration Grant Program
This program is designed to address unhealthy behaviors in at-risk minority youth, and provide them opportunities to learn about more positive lifestyles and enhance their capacity to make informed and healthier life choices. It is intended to test community-based interventions on reducing risky behaviors among targeted minority youth. These demonstration grants require a multi-partner approach involving institutions of higher education, primary and secondary schools, community organizations and institutions, and the community at-large.

This program seeks to improve the health status relative to HIV/AIDS, of targeted minority populations through health promotion and education activities. This program is intended to test community-based interventions on reducing HIV/AIDS disparities among racial and ethnic minority populations, and demonstrate the effectiveness of community-based partnerships involving non-traditional partners at the local level.

This program is designed to address the many and significant health disparities faced by rural disadvantaged and minority populations throughout the state. This program is intended to demonstrate the effectiveness and efficiency of a targeted and multifaceted statewide approach for eliminating these health disparities.

- State Partnership Grant Program to Improve Minority Health
As part of a larger national strategy, this new initiative seeks to facilitate the improvement of minority health and elimination of health disparities (adult/child immunization, asthma, cancer, diabetes, heart disease and stroke, HIV, infant mortality, and mental health) through the development of partnerships with established state and territorial offices of minority health. Grant recipients will identify community-based minority-serving organizations that are well linked with minority populations affected by HIV/AIDS, and which have recognized needs and/or gaps in their capacity to provide HIV/AIDS-related prevention and care services. This will be facilitated by providing administrative and programmatic technical assistance; and assisting those community-based minority-serving organizations, through an ongoing mentoring relationship, in the development of their capacity as fiscally viable and programmatically effective organizations to successfully compete for federal funds and other resources.

COOPERATIVE AGREEMENTS
Cooperative Agreements are administered by the Office of Minority Health (OMH). Cooperative agreements support research, demonstration and evaluations, test new and innovative models, increase knowledge and understanding of health risk factors, and develop mechanisms that support better information dissemination, education, prevention, and service delivered to individuals from disadvantaged backgrounds.
There are two types of cooperative agreements administered in OMH, standard and umbrella. The standard cooperative agreement has a single scope of work similar to a grant. The umbrella cooperative agreement provides a broad programmatic framework in which specific projects can be supported by various government agencies during the project period.
- HIV/AIDS Health Promotion and Education Cooperative Agreement Program

These grants are aimed at increasing community understanding of HIV/AIDS and improving access to HIV/AIDS counseling, testing, and treatment services. The involve coalition efforts to promote integrated community responses; address sociocultural, linguistic, and other barriers to effective HIV prevention and services, to increase the number of individuals seeking and accepting treatment; and develop and conduct education and outreach efforts for hardly reached populations.
Fact Sheet - Family and Community Violence Prevention Program
The Family and Community Violence Program (FCVP) was developed in fiscal year 1994 under a cooperative agreement between OMH and Central State University (CSU), Wilberforce, Ohio. The purpose of the FCVP is to impact the increasing incidence of violence and abusive behavior in low income, at-risk communities through the mobilization of community partners. The FCVP coordinates the activities of Family Life Centers (FLCs) at Historically Black Colleges and Universities, Hispanic Serving Institutions, Tribal Colleges and Universities, and other minority-serving institutions in 17 states, the District of Columbia, Puerto Rico and the U.S. Virgin Islands. The FLCs are engaged in violence prevention activities which address the academic, personal, cultural and career development of youth who are at risk for involvement in violent and other abusive behavior. Each FLC provides a summer academic enrichment program and conducts activities to improve family bonding, parenting skills and social interaction skills. Each has collaborative agreements with schools and community organizations to assist in recruiting youth and their families and delivering services.

- Community Initiatives to Eliminate Stroke Program (CITIES)
The Community Initiatives to Eliminate Stroke Program (CITIES) is intended to increase community awareness and knowledge of hypertension and stroke, enhance early detection of high blood pressure and stroke with early referral to care, increase the community's adoption and use of lifestyle behaviors known to promote prevention and control of hypertension and stroke, reduce the death rate by stroke in targeted communities, and enhance blood pressure control rates among community persons who are known to have hypertension and who are members of a health plan or otherwise visit health systems, clinics, or medical offices. The three grantees are: Medical University of South Carolina, Charlestown, SC; Forsyth Medical Center Foundation, Winston-Salem, NC; and University of Alabama at Birmingham, Birmingham, AL. (2005)
Fact Sheet - National Umbrella Cooperative Agreement Program
The National Umbrella Cooperative Agreement Program seeks to increase the diversity of the health-related work force, reduce health disparities and improve quality of care for targeted minority populations through projects that are of national significance, and improve evaluation procedures and the collection and analysis of data on targeted minority populations. This program offers a mechanism for the Department and other Federal agencies to colaborate on projects addressing these purposes. Nine organizations were awarded cooperative agreements in FY 05, four of which focus on health disparities projects and five on work force development projects.
