The Massachusetts Medical Home Network Project
This educational program of New England SERVE brings together primary care providers, parents and community partners to demystify the theory and practice of the Medical Home. Its case-based curriculum offers knowledge, awareness, and strategies for implementing small practice improvements. The program includes a series of three case-based discussions of caring for children with ongoing health conditions or disabilities. The goal of the curriculum is to increase the capacity of primary care physicians to care for CSHCN.

    Sessions include:
  • Introduction to Medical Home, CSHCN, Family Centered Care
  • Community Resources and Care Coordination
  • Medical Home Index and Practice Change

These two-hour sessions carry continuing medical education (CME) approval.

Ongoing Medical Home Training Academy
A Medical Home Academy (MHA) was presented on March 8, 2005 at the Hartford Marriott in Rocky Hill, CT, in collaboration with the Child Health & Development Institute of Connecticut, University of Connecticut Health Center and Eastern Area Health Education Center. This continuing education program is designed to educate pediatricians, family physicians, other health care providers, family representatives, child health advocates, hospital ambulatory care for families, community health centers, school-based health centers, insurance administrators, government officials, and any other group of individuals that work with children and youth. Over 100 individuals participated in this inaugural event.

The MHA is part of an continuing effort to establish a quality ongoing primary care practice improvement training system for pediatric and adolescent health care providers in order to improve the holistic and comprehensive health of children, youth and including those with special health care needs. The MHA presented the practical and effective practice improvement methodology that is needed to enhance primary care for children and their families/caregivers in Connecticut.

Existing national and statewide curriculum were modified into Connecticut-specific training modules to first be delivered in the one-day conference. Additional sessions will be offered in a variety of formats including pediatric office in-service trainings, Grand Rounds, CD’s, and video & audiotapes.

The modules include:

  • Family-Professional Partnerships
  • Developmental Health Screening & Surveillance
  • Transitioning Children & Youth to Adulthood
  • Medical Home: Next Steps

Immunization in a Medical Home Training Tool
For 75 years, the American Academy of Pediatrics (AAP) has been a recognized leader and resource for information on childhood immunization. In an effort to achieve a system-wide improvement in the immunization delivery system, the AAP Childhood Immunization Support Program (CISP), in partnership with the National Center of Medical Home Initiatives for Children with Special Needs, has developed a training tool to assist health care professionals to immunize children in the context of a medical home.

The CD-ROM based training tool, “Immunization in a Medical Home,” is comprised of a 150+ page Power-Point presentation that:

  • Relates the 7 components of a medical home – accessible, family centered, continuous, comprehensive, coordinated, compassionate, and culturally effective – to vaccine administration and management
  • Offers practical strategies to ensure that immunizations are provided in a medical home
  • Provides in-depth immunization case study reviews that demonstrate core medical home concepts

Supplemental materials include fact sheets on various immunization topics, AAP policy statements, reference articles, and immunization resources.

The training tool is designed for use by pediatricians, pediatric health care professionals, immunization training programs, pediatric residency programs, and others with an interest in immunization, and is ideal for presentation at meetings, conferences, or other educational venues. The training tool can be used as a stand alone session or divided up into shorter training sessions, enabling customization of program length and target audience. New Online Transition Training for Case Managers

The training is accredited continuing education for social workers, nurses, professional counselors, and educators. There is a $10 fee to obtain the CEUs, but ANYONE may go to the web site and receive ALL of the information FREE. Most of the information has broad application and targets health care transition, with only limited portions of the information pertaining to services and resources available in Texas.

E-adolescence
