Medical Home Training Materials

 
The national framework of the curriculum’s content provides great flexibility to accommodate the varying degrees of audience familiarity with the medical home concept. This format allows the materials to be customized with local information to fit a community’s unique needs. Seven components comprise the curriculum which enables it to be presented all at once in a 1-day program, individually as shorter training modules or separate sessions at local, naturally occurring meetings. Below are each of the components to view or download, as well as each component in a slide show format.

Each component is available in faculty and participant versions, and has appendix items that supplement its topic area. However, while both formats present the same content and resources, the faculty version additionally offers teaching strategies to assist in the training process.

The materials are available in Word and PDF format. The Reader is available free from the Adobe web site.

    Overview of the medical home approach to quality, comprehensive health care for children with special needs.
  • Family-Professional Partnerships
    The common elements of family-centered care and strategies to develop family-professional partnerships.
  • Practices, Policies and Procedures
    Practical strategies to implement medical homes into practice.
  • Comprehensive, Coordinated, Collaborative Care
    Strategies for coordinating care, working with community based resources, and collaborating with specialists and allied health care professionals.
  • Transitions
    Understand transition related issues and strategies for advancing children and youth into adulthood.

    Learn strategies to better advocate for children with special health care needs and identify key partners to assist in these efforts.
  • Surveillance and Screening
    Understand the importance of implementing continuous surveillance within a medical home, while learning about practical screening tools to utilize if needed.
    Offers discussion regarding mental and oral health, developmental, vision and hearing screening.

Case Studies
Below are downloadable files that contain case studies for use with the training curriculum. Case studies provide participants with a practical and applied learning opportunity during the training sessions. Each file contains 3 problem based learning scenarios with discussion points and facilitator guidelines.
