What resources are available to
assist with training initiatives?
- A nationally written curriculum on medical home, comprised
of seven components, available in English and Spanish
- A curriculum that allows for localization of content
and
flexibility in program schedule
- Resources and tools for practical implementation of
medical
homes in communities, practices and families
- Facilitator materials to enable providers and parents
to lead
trainings: slide shows, teaching strategies, group exercises
- A faculty database to locate facilitators that match
the intent
of your local training program
- A How to Host Guide that walks you through the process
Technical Assistance available
to assist you in beginning medical home training initiatives
in your community or practice.
The following are available
at no cost:
- Master copy of training curriculum, power point slides,
and marketing materials
- Ongoing technical assistance throughout the planning
process, including consultation at planning meetings
- AAP Web site space to promote medical home training
programs in your community
- Assistance in obtaining CME credit
- Assistance in identifying speakers
- Venue and funding search
